For a copy of your medical records:
- Please click the link below to submit a medical records request; Do NOT submit your request in person.
- There is no charge for your records.
- You should receive your records within 5 business days of completing your request.

CD/IMAGING REQUESTS: To request a CD, please call 406-586-8029 and speak to any of the front office staff to put in your imaging request. The CD can be picked up in office or mailed.
